2017 Annual Optimal Aging Conference | Conference AwardsThe UofL Institute for Sustainable Health & Optimal Aging and the Kentucky Association for Gerontology (KAG) are pleased to announce its Awards and Recognition Program. The program has been created to: a) recognize the excellence of the personal efforts of individuals working in the fields of gerontology and geriatrics; and b) bring increased public and professional attention to and interest in gerontology and geriatrics across the Commonwealth of Kentucky. There are four different types of awards for which individuals can be nominated:

You may attach up to two typewritten pages of supporting information in addition to completing the Nomination Information section. You may single space and use ½ inch margins, but do not use less than 12-point font. If you are a KAG member you do not need a Second nominator.
