2017 Annual Optimal Aging Conference | Conference Awards



The UofL Institute for Sustainable Health & Optimal Aging and the Kentucky Association for Gerontology (KAG) are pleased to announce its Awards and Recognition Program.

The program has been created to: a) recognize the excellence of the personal efforts of individuals working in the fields of gerontology and geriatrics; and b) bring increased public and professional attention to and interest in gerontology and geriatrics across the Commonwealth of Kentucky.

There are four different types of awards for which individuals can be nominated:

  • The Lois E. Layne Award (for student achievement)
  • The Thoroughbred Award (for exceptional advocacy for seniors)
  • The Distinguished Achievement Award (Educator Category)
  • The Distinguished Achievement Award for practitioners (Practitioner Category)

Detailed descriptions and criteria for each award, along with nomination instructions, are available in the Conference Awards Information & Nomination Packet.

How to Nominate:

To nominate an individual for one of the Optimal Aging Conference awards, please submit three pieces of information

  1. Complete the Nomination Information section found at the end of the Conference Award Instruction & Nomination Packet.
  2. Provide brief but explicit summary of nominee’s efforts and accomplishments.
  3. Address and number each of the Award Evaluation Criteria for the respective award for which you are submitting a nomination.

You may attach up to two typewritten pages of supporting information in addition to completing the Nomination Information section. You may single space and use ½ inch margins, but do not use less than 12-point font.  If you are a KAG member you do not need a Second nominator.
